PPO Insurance


Dr. Lyass  accepts most PPO insurance plans. Patients with PPO insurance can see him without needing a referral or authorization.
  PPO Insurance Approval Process - Obtaining an insurance approval for weight loss surgery can be a difficult process. The first thing we do is call your insurance company for two things: 1) to verify that weight loss surgery is a covered benefit, and 2) to see what the exact requirements are to obtain authorization for surgery. We will then help you to meet the necessary requirements.

  PPO Out of Pocket Costs - Insurance companies generally don't pay for all the costs associated with having surgery. Out of pocket costs can include things like deductibles, co-payments and co-insurance. We will inform you of what your out of pocket costs would be after we have called your insurance company to check your benefits.

HMO Insurance

HMO patients need to be referred by their primary care doctor to see us. Your primary care doctor may or may not be able to refer you to us, depending on whether or not we have a contract with your HMO. Call our office to see if we are contracted with your HMO and to see what the insurance approval process entails.
